Insurance & Payment Options
Looking for a Bay Area dietitian who accepts insurance? I offer one-on-one nutrition counseling with insurance and self-pay options, so you can choose the payment approach that works for you.
Accepted insurance plans
- Aetna
- UnitedHealthcare (UHC)
- Blue Cross Blue Shield (BCBS)
- Medicare
Coverage for nutrition counseling or medical nutrition therapy depends on your specific plan and benefits. Acceptance of your insurance does not guarantee that every appointment is covered.
Check your coverage before your visit
Call Member Services using the number on the back of your insurance card. Ask:
- Does my plan cover nutrition counseling or medical nutrition therapy for my needs?
- Is Sneha Jain, RD in network under my specific plan?
- What cost-sharing, deductible, copay, or coinsurance will I be responsible for?
- Are there visit limits, prior authorization requirements, or documents I need to provide?
A PCP referral is required for medical nutrition therapy at this practice, even if your insurance plan does not require one. Your provider can download the referral form and find fax instructions.
Self-pay & HSA/FSA cards
Self-pay appointments and HSA/FSA cards are accepted. Get in touch for current appointment fees and payment details. Check with your HSA/FSA administrator about eligible expenses and documentation.
Out-of-network insurance & superbills
If I am not contracted with your insurance plan, I can provide a superbill for you to submit to your insurer for possible out-of-network reimbursement. Ask your insurer about these benefits before your appointment; reimbursement is determined by your plan.
Nutrition support that fits your life
My approach combines assessment and practical strategies with follow-up support for meal planning, troubleshooting, and accountability. I specialize in vegetarian, Indian, and South Asian nutrition and welcome clients from all backgrounds and eating patterns.
